Since we have this very very small list of statuses, can we force a status to require an extension?

Yes, you can. Beyond the predefined 10 statuses there's indeed the Status Ext. field. You can configure it to be mandatory, as well as for other fields by visiting Tools > Options > Tickets (Admin) tab, and clicking the 'Setup Mandatory Fields' button (top/right of the tab).

This looks like its only when making new tickets. What I am looking for is making it mandatory that using a certain status must include an extension.

This setting forces using the Status Ext. field when new Tickets are created. I'll add a feature request to support variants on a per-status level and the option to configure that only specific statuses require using an Ext. Thanks.
