I need to automatically generate some tickets every month, connected with maintenance schedule. I can generate recurring task and appointments, and add associated additional items, but only once.

Basically, you are going about this in the right way. You may set recurring Tasks or Appointments (optionally with reminders) with all the details about the recurring activity inside the Task's/Appointment's description. These will remind you to perform the maintenance activity.

When the time is due, you can easily right-click the Appointment/Task and select to add a new Ticket out of Appointment/Task. This will automatically link the new Ticket to the Account and copy the activity description to the Ticket's description. All you need to do now is select the relevant Contract (in case you use a special contract for maintenance) and you are done, the Ticket is ready for work.

On a side note, if you use special Contracts for maintenance, you may keep default Items per Contract to be used whenever you perform work for this Contract, which will speed-up the process of adding Charges for this Ticket later on. This can be done in the Contracts' window in the Details tab.
