Having trouble embedding login page on website

I copied the HTML code from the wiki to a page on our website and changed the IP/URL and port number as per the instructions. However, when trying to log in, you are taken to a page that says the connection is refused. Has anyone had a similar problem? I'm not sure what I can change on my end to remedy this.
