Migrate to a new Server

We are upgrading our infrastructure from Windows 2003 Std, to Windows 2008 Ent. We have the CommitCRM SQL option.

What procedures do I need to follow to migrate CommitCRM to the new server? Once migrated over the old server will retired.

It's been several years since we installed CommitCRM and the SQL so I'm not sure if we still have the activation information for it.
